The Assessment of Primary Care Resources and Supports for Chronic Disease Self Management (PCRS) is a tool developed by the Robert Wood Johnson Foundation Diabetes Initiative to further define the elements of self management support in primary care. Its purpose is to facilitate quality improvement in this component of the chronic care model.  Also available is a PowerPoint presentation that provides some background and information on how to use and score the PCRS Tool.

Electronic Medical Records/Health Information Technology
HRSA’s Office of Health Information Technology (OHIT) has developed Electronic Health Records (EHR) Selection Guidelines to assist health centers evaluate EHR products and develop requests for proposals (RFP) or requests for information (RFI).  The Guidelines are customized for health centers and were developed in consultation with IT staffers and clinicians from health centers.

 The Guidelines provide the following types of assistance:
§         Allow health centers to gather detailed information from vendors as they consider EHR products;
§         Can be used by health centers to specify EHR needs to vendors as part of an RFP 
§         Can be used to evaluate vendor proposals and products to make informed purchasing decisions
§         Represent a relatively comprehensive list of features for evaluation of EHR products but can be added to or subtracted from to make sure the EHR you select meets your needs

2.   HRSA HIT Community Portal-  HRSA’s OHIT has developed a HRSA Health IT Community as a tool/resource center for health centers that serves as a virtual community for health centers, networks and PCAs to collaborate around the adoption of technologies promoting patient safety and higher quality of care.  The goal is to facilitate collaboration among health centers via discussion forums where health centers facing similar challenges can share thoughts and lessons from experiences with a variety of IT systems and scenarios.  If you have not joined, please email Christie Brown at Christie.brown@hrsa.hhs.gov to receive a login ID.
