Workers and Health: Community Health Centers Making a Difference in the Protection of Migrant Workers and Their Families

This poster will highlight one of MCN’s Centers of Excellence in North Carolina, Blue Ridge Community Health Services (BRCHS). Their partnership emphasizes worker health to all staff and incorporates an environmental and occupational medicine focus into clinical care.

Advanced Practice Clinician Safety Net Residency
Focus Areas: Operations, Clinical
Through the Advanced Practice Clinician Residency Program, new graduates from PA or NP programs are selected for their desire to practice in a safety-net setting.  The 12-month program allows for a slow build-up of patient panels in a collaborative environment where MD's, NP's, and PA's work side-by-side.

An estimated 70 million adults at the highest risk for pneumonia remain unvaccinated. The objective of this project was to implement a vaccination strategy in a free-clinic pharmacy and to demonstrate the importance of the role of pharmacists in improving our patients’ access to preventive care.

Integrating Pediatric Medical Visits with Oral Health using a Dental Liaison

Gaston Family Health Services, Inc. employs a dental liaison to work within their Child Health clinic to educate parents and children on oral health and to make referrals to the dental clinic as needed.  If a parent has any special concern or request, she communicates directly with the dentists or dental staff.

Through cooperation with CareNet Counseling and the Office of Rural Health and Community Care, Grace clinic has been successful in implementing an integrated care model using a pastoral counselor in a residency program to assess all patients for behavioral health needs and to provide therapy as needed.

Impact of Comparing Stand-Along Part D Drug Plans for 2013 & the Role of the Pharmacist in Drug Plan Selection

Focus Areas: Quality, Clinical, Outreach/Special Populations
Patients at Community Health Centers and Free Clinics are often eligible for federal subsidies to help pay for Medicare Part B or Part D. Local SHIIP coordinating sites can help with comparing plans, participating providers, and covered medications annually in order maximize savings for Medicare beneficiaries.
