Carolina Medical Home Network – Accountable Care Organization (CMHN-ACO) was a partnership of four NC health centers and NCCHCA that entered into the Medicare Shared Savings Program (one-sided model). CMHN-ACO received funding from the Center for Medicaid and Medicare Services (CMS) for ACO Investment Model (AIM) to support care coordination efforts at ACO member health centers and network administrative services. The grant period for the ACO ended in 2020.
NCCHCA launched a data-informed outreach project in collaboration with CMHN that supports community health workers in North Carolina health centers to augment care coordination efforts.
