CARE COORDINATOR I

Freedom Hill Community Health Center is hiring for a Care Coordinator I!

This position supports the Mission, Values, and Vision of Carolina Family Health Centers, Inc. The Care Coordinator I will assist the medical providers in providing comprehensive medical care to patients through care coordination and population health.

Minimum Qualifications:
High school diploma or GED.
Certified Nursing Assistant.
Valid NC Driver License.
BLS certification.
Basic computer skills.
Must be detail and team oriented.
Effective communication and interpersonal skills.
Must maintain a high degree of unquestionable integrity in protecting confidential patient information.
Physical Requirements:
Individuals may need to sit or stand for long periods, occasional bending, stooping, lifting including patients, reaching forward, and overhead.
May require walking primarily on a level surface for periods throughout the day.
Proper lifting techniques are required.
Frequent computer work is required.
Must be able to travel freely in the local area; between clinics, hospitals, health departments, etc. Personal vehicle required.
