Job Description:


The Community-Centered Health Program Coordinator (CCHPC) serves as the project lead for NCCHCA’s Community-Centered Health (CCH) grant, a project administered by NCCHCA in collaboration with a coalition of partners, including Piedmont Health Services, Student Action with Farmworkers, the NC Farmworker Health Program, Farmworker Advocacy Network, and NC Farmworkers’ Project. The CCH grant aims to create systemic changes related to the occupational health hazards and chronic illnesses which disproportionately impact current and former farmworkers and their families across North Carolina

Qualification Requirements

  • Bilingual in English and Spanish.
  • Experience working with current and former farmworkers and their families, and an understanding of the social determinants of health that impact farmworkers.

Minimum Qualifications

BA or BS degree in an associated field, such as Public Health, Health Education, Nursing, or Social Work and two (2) years of relevant work experience.

  • Ability to provide leadership, make difficult decisions, and be consistent and calm during crises.
  • Ability to effectively develop relationships and partner with a diverse group of community members, organizations, and other stakeholders.
  • Ability to use initiative and independent judgment within established procedural guidelines, prioritize ongoing and deadline-based tasks, and work with shifting priorities and schedules.
  • Outstanding oral, written and interpersonal communication skills, including the ability to make presentations to various audiences.
  • Extremely organized and a self-starter.
  • Strategic thinker with strong interpersonal skills, including ability to work as a team member and leader.

Supervisory Responsibilities

  • None.

Essential Functions

Grant Administration

  • Establishes and maintains a collaborative relationship among all CCH Core Team members, broader coalition partners, and NCCHCA staff to promote cohesion and quality assurance of the CCH grant.
  • Coordinates and oversees programmatic purchases, sub-contracts, and other expenses.
  • Manages evaluation methods for tracking grant outcomes and establishing programmatic benchmarks.

Prepares reports and provides regular project updates to NCCHCA board members, funders, technical assistance provider, Core Team members, and partnership stakeholders.

Project Management

  • Manages the day to day logistics and operations for project meetings, activities, and events.
  • Facilitates training for and meetings between Core Team members, broader coalition partners, and appropriate NCCHCA staff.
  • Organizes and facilitates coalition development and capacity building for the project.
  • Assists with implementation of community needs and resource assessments and surveys as appropriate for future initiatives and funding.
  • Maintains and utilizes a working knowledge of significant developments, trends, and best practices in the field of farmworker health.
  • Attends trainings and researches local and national trends, including local, state, and federal policy related to agriculture and farmworker health.

Program Development

  • Develops a Community-Centered Health implementation plan built through community engagement and a structured development process with support from Core Team members and other coalition partners.
  • Facilitates and supports the engagement of current and former farmworkers and their families in developing project strategy.
  • Collaborates with local non-profits, government agencies, and other relevant partners to promote collaboration, negotiation, sharing of best practices, problem-solving, and project sustainability.
  • Engages and builds relationships with local and statewide policy makers to advocate for policies and practices that support farmworker community health and wellness.
  • Identifies needed systems changes and drive partnership toward implementation.
  • Looks for opportunities to engage new community partners and build coalition membership.
  • Maintains connections with other NC coalitions and looks for opportunities to collaborate on projects and bring attention to agricultural health and well-being.

  • regularly required to, stand, sit; talk, hear, and use hands and fingers to handle or feel; and reach with hands and arms to operate a computer and telephone keyboard, and to reach, stoop or kneel
  • Specific vision abilities required by this job include close vision requirements due to computer work
  • Light to moderate lifting is required such as occasionally lifting office products and supplies, up to 20 pounds
  • Ability to uphold the stress of traveling
  • Regular, predictable attendance is required; including quarter-driven hours as business demands dictate.

Work Environment

  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
  • Level of noise typical for the work environment of this job is Moderate – less than 85 decibels (business office with computers and printers and traffic within the receptionist area)
  • Ability to work in a confined area.
  • Ability to sit at a computer terminal for an extended period of time.

Preferred education, skills and experience

  • Master’s degree.
  • Grants management experience.
  • Statewide coalition building experience.
