POSITION: Community Health Navigator (serving Alleghany County)

LOCATION: Based at the Alleghany office of AppHealthCare; will routinely travel as work priorities demand.
SALARY RANGE: $30,000-32,616
JOB DESCRIPTION: The Alleghany Community Health Navigator (CHN) will assist individuals in Alleghany County NC in accessing mental health, substance use, intellectual/developmental disability services, within Alleghany County. The CHN will work closely with county stakeholders to support Alleghany County residents in accessing needed services and will work closely with AppHealthCare Community Health Worker and others to develop the NCCARE360 platform to assist linking individuals to services in Alleghany County.
This will involve a coordinated effort with county and community services to assure individuals are identified and appropriately connected to services. The position will support those transitioning from detention or treatment facilities and those experiencing homelessness, and will enlist the aid of peer support, pastoral ministries, municipal/community organizations and other agencies/providers who support and assist persons who suffer from substance use and/or mental health challenges.
KNOWLEDGE, SKILLS, ABILITIES: Previous experience with substance use and mental health treatment and resource navigating is required. Ability to communicate and identify effectively people who use drugs and in community groups, distributing general information, and education related to substance abuse and mental health. Ability to develop effective working relationships with professionals involved in treating substance abuse.
ADDITIONAL KNOWLEDGE, SKILLS, & ABILITIES PREFERRED:
This position will require strong coordination and organizational skills, and considerable experience in outreach efforts.
Relationship building and strong leadership skills are a must.
REQUIREMENTS:
A valid driver’s license and automotive insurance are required. A minimum of two continuous years in recovery from any mind-altering substances, not including medications prescribed to you. Certification as a NC Peer Support Specialist is required, in progress is also acceptable.
MINIMUM EDUCATION:
High school diploma or GED or equivalent is required. At least one year of experience working with individuals with substance use disorder and/or their families in a supportive informational role is required; or an equivalent combination of education and
experience.
