Community Health Worker
Western North Carolina Community Health Services is seeking a full-time Community Health Worker (CHW). The CHW is a community-based position that functions collaboratively with nurse care managers, care coordinators, population health specialists and others to identify and outreach to hard-to-reach patients.
The CHW helps to coordinate patient care and assists patients with accessing healthcare and community resources. Through concerted and targeted outreach efforts, the Community Health Worker helps to monitor patient adherence to treatment plans and close gaps in care by identifying and addressing social support needs, literacy limitations, and other social determinants of health impacting access to services.
This position requires superior communication and interpersonal skills with a wide and diverse range of patients, staff, physicians and public. One year experience working with the public, or in customer service; in a medical setting is preferred. Knowledge of medical office operations and familiarity with role of FQHCs in the community is preferred.
