Carolina Family Health Centers, Inc. is hiring for a PART-TIME LICENSED PRACTICAL NURSE OR REGISTERED NURSE / CASE MANAGER I – POPULAITON HEALTH – to work with the Population Health Team at Wilson Community Health Center.

This part-time opportunity is 20 hours a week, 1:00 to 5:00 p.m. Monday through Friday, and is eligible for prorated leave and holidays, including the participation in the 401(k) retirement plan.

The Case Manager I will provide medial case management services to a specified panel of patients within the clinical setting.

The Case Manager I will perform comprehensive assessments to evaluate the patient’s health care needs and social determinants of health to develop individualized and person-centered care plans, and monitor and re-evaluate the plan as necessary. Also, identify patients with recent emergency department visits or hospital admission, and assist them with transitioning back to their primary care provider including medication reconciliation and close gaps in care.

MINIMUM QUALIFICATIONS:
Graduate of an accredited school of nursing as a Licensed Practical Nurse or Registered Nurse.
Current and unrestricted license to practice in North Carolina.
Previous experience, preferably, in a community health center setting in the area of nursing and/or case management.
Valid North Carolina driver’s license.
Must have personal vehicle readily available for frequent travel between hospital, patient’s home, and site-to-site visits.
Effective communication and interpersonal skills.

PHYSICAL REQUIREMENTS:
Frequent sitting, standing, walking, occasional bending, stooping, lifting, reaching forward and overhead.
Frequent computer work required.
Must be able to travel freely in the local area.
