Job Description:

Position Summary
The Care Management (CM) Manager is an RN who will oversee the day-to-day operations of the Care Management team, including the RN Care Managers and the Care Coordinators. The CM Manager will serve as CM Lead and the care management contact for CMHN (Carolina Medical Home Network), along with the point of contact for all internal CM/CC work. The CM Manager will review performance metrics and work with leadership and providers to provide CM/CC services to NHC patients.

Principal Duties and Responsibilities*
Care Management
– Oversee the day-to-day operations of the Care Managers and Care Coordinators on the CM team
– Serve as the care management contact for CMHN
– Actively participate in the Care Management Leaders workgroup
– Communicate CM program updates to staff
– Collaborate with partner organizations to provide the best care and resources to our patients as needed
– Review performance metrics and work with leadership, staff, and providers to improve workflows and efficiencies in accordance with CMHN and NHC protocols.
– Serve as a Care Manager to support the workload of the Care Managers as needed.
– Serve as the point of contact for grant programs supporting care management work.

Strategic Leadership
– Responsible for the financial management of the CM team, including providing input for budget preparation and ensuring that costs are well-controlled for the CM team.
– Participate in strategic planning, goal setting, writing and implementation of procedures; and ensuring compliance with and achievement of those goals and procedures.
– Promote growth of CM staff in areas of leadership, operational excellence, patient-centered care, education, and organizational involvement to meet the identified goals and needs of the employee and organization.

Management
– Work with Human Resources in the hiring, onboarding, and offboarding of department team members.
– Direct the training of newly hired team members and deploy training, retraining, and skills documentation to maintain the required performance of the team.
– Hold routine department meetings to ensure efficient workflow of the clinic/department.
– Receive input from, review, and report on the performance of team members.
– Ensure compliance with federal, state, and local regulations, including OSHA, HRSA, and HIPAA regulations.
– Solve conflicts between staff members and resolve patient complaints.
– Increase employee motivation, retention, and engagement by providing ongoing coaching; promote teamwork within and outside of the clinical area.
– Function as troubleshooter regarding staff issues and proactively establish lasting change and procedures to prevent future issues.

Required Skills or Abilities*
1. Excellent verbal communication, organizational, problem-solving, and critical thinking skills.
2. Experience with an electronic medical record.
3. Ability to organize, direct, prioritize, and delegate work appropriately.
4. Computer literacy in internet use and Windows environment, including Outlook, Word, and Excel with keyboarding skills of at least 45 wpm.
5. Requires intermediate understanding of Excel and how to gather and present data.
6. Requires strong interpersonal communication and mediation skills to successfully lead, motivate, and collaborate with a very diverse staff in a variety of capacities.
7. Ability to strongly embrace and personify the mission of the NHC, through performing work in a whole person way, providing for the physical, emotional, and spiritual needs of staff members and patients.
8. Ability to cultivate and develop inclusive and equitable working relationships with co-workers and community members.
9. Ability to serve as an advocate for individuals of all ethnicities, genders, ages, and backgrounds.

Required Knowledge, Experience, or Licensure/Registration
1. Bachelor’s Degree from an accredited School of Nursing with an unrestricted RN license to practice nursing in North Carolina.
2. Minimum of 5 years of nursing experience in an ambulatory or acute care setting, home health, or public health.
3. Experience in Care Management is required
4. Leadership or management experience is highly preferred.
5. Experience and demonstrated abilities of working in a multi-cultural setting.
6. Ability to work onsite/hybrid/remotely Mondays through Fridays during the hours of 8am and 5pm.
7. Ability to work remotely via a home office set up with access to secure Internet connection.
8. Ability to read, write, speak, and comprehend English fluently.
9. Routine immunizations and flu vaccines
