Job Description:

Appalachian Mountain Community Health Centers (“AMCHC”), a Federally Qualified Health Center network based in beautiful Asheville, NC, is seeking to hire a Physician Assistant or Nurse Practitioner to join our exceptional clinical team in delivering quality-focused primary care services for an adult (16+) patient base at our Asheville and Sylva-based clinics.

AMCHC provides comprehensive, accessible, patient-centered primary care to all, including low-income, medically underserved and uninsured populations, at six locations throughout Western North Carolina.

Key accountabilities for the PA/NP position include:

Working closely with physicians, other advanced practitioners, behavioral health clinicians, and clinical and administrative support staff in a team-based approach to patient care.

Assessing patient status; evaluating presenting illness, risk factors, family history, psychosocial, economic and cultural factors; performing appropriate physical examination; and integrating data to determine diagnosis and therapeutic plan, including identification of any health risks.

Developing and implementing treatment plans, including prescribing and/or dispensing medications and injections in compliance with best medical practice and applicable state and federal regulations.

Preparing documentation for the Electronic Health Record (EHR), including progress notes, diagnostic laboratory and radiology results, medications, diagnoses and treatment plan(s)
Facilitating and participating in shared decision making with patients, providing health education and promotion and recommending or referring patients to appropriate community resources to address social determinants of health
Promoting continuity of care between visits according to the patient’s treatment plan, including triaging patient calls.

Operating from a framework of continuous improvement in all aspects of practice, including standard operating procedures, compliance, and practice management.

Maintaining focus on and achieve target metrics for patient encounters, quality and patient satisfaction.

Working with and providing appropriate guidance to clinical support staff, including nurses and medical assistants.

Required qualifications include:

PA/NP degree from an accredited institution.
Current PA-C or NP license.
Board certification.
CPR certification.

Three (3)+ years of practice experience with an adult population in a public health setting, preferably in a rural or under-served area.

X-Waiver and experience treating patients with Substance Use Disorders strongly preferred.

Strong commitment to practicing in a community health setting.

Excellent interpersonal skills, as well as critical thinking skills, the ability to handle stressful situations and the capacity to function independently.

Skill in establishing and maintaining effective working relationships with colleagues, staff, community resources, patients and the public.

Ability to organize work, delegate and achieve goals and objectives
High degree of initiative, judgment, discretion and decision-making to achieve AMCHC’s mission.
