POPULATION HEALTH MANAGER – WILSON COMMUNITY HEALTH CENTER

Wilson Community Health Center has an opening for a Population Health Manager. The Population Health Manager will develop, monitor, and maintain patient care management and transition of care activities at Carolina Family Health Centers, Inc. Provides guidance and supervision to the Case Management Team while maintaining a caseload of patients. Serve as point of contact for local hospitals in order to assist with patient discharge and overall program goal to improve health outcomes and reduce hospital 30-day readmission rates and emergency room utilization.

Minimum Qualifications:
Graduate of an accredited school of nursing. Licensed as a Registered Nurse in the State of North Carolina.
Three years in a healthcare environment with supervisory/management background and experience in case management.
Must have personal vehicle readily available to freely travel in the local area; between clinics, hospitals, health departments, etc.

Physical Requirements:

Individuals may need to sit or stand for long periods of time, occasional bending, stooping, lifting including patients, reaching forward and overhead. May require walking primarily on a level surface for periodic periods throughout the day. Reaching above shoulder heights, below the waist or lifting as required up to 50 pounds. Proper lifting techniques required. Frequent computer work is required.
