Registered Nurse, Case Manager II – Ryan White Program

REGISTERED NURSE, CASE MANAGER II – RYAN WHITE PROGRAM – WILSON COMMUNITY HEALTH CENTER

Carolina Family Health Centers, Inc., is hiring for an RN, Case Manager II – Ryan White Program.

This position supports the Mission, Values, and Vision of Carolina Family Health Centers, Inc. Under the direction of the primary care provider, the Registered Nurse is responsible for providing medical case management services to a specific panel of patients within the clinical setting. This position will also perform Medicare Annual Wellness Visits (AWV). This position reports directly to the Ryan White Program Manager.

Minimum Qualifications:
Current Registered Nurse license.
BLS Certification.
Previous experience in a community health center setting in the area of nursing and/or case management is preferred.
Active North Carolina Driver License.
Effective communication and interpersonal skills.
Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.
Strong computer skills and working knowledge of the windows-based environment.
Must have experience with electronic health record.

Physical Requirements:
Frequent sitting or standing for long periods of time.
Occasional bending, stooping, lifting including of patients, reaching forward and overhead. Proper lifting techniques required.
Walking primarily on a level surface for periods throughout the day.
Frequent computer work required.
Must be able to travel freely in the local area.
As a Medicaid/Medicare facility, Carolina Family Health Centers must comply with the Federal COVID-19 vaccination mandates.
