REGISTERED NURSE, CASE MANAGER II – POPULATION HEALTH – WILSON COMMUNITY HEALTH CENTER

Carolina Family Health Centers, Inc., is hiring for a Case Manager II.

This position supports the Mission, Values, and Vision of Carolina Family Health Centers, Inc. Under the direction of the primary care provider, the Registered Nurse is responsible for providing medical case management services to a specific panel of patients within the clinical setting. This position will also perform Medicare Annual Wellness Visits (AWV). This position reports directly to the Population Health Manager.

Minimum Qualifications:
Current Registered Nurse license.
BLS Certification.
Previous experience in a community health center setting in the area of nursing and/or case management is preferred.
Ability to travel 50% of the time to either Harvest Family Health Center (Elm City, NC) or Freedom Hill Community Health Center (Tarboro, NC).
Active North Carolina Driver License.
Effective communication and interpersonal skills.
Must demonstrate the skills and judgment necessary to provide direct care to patients and nursing interventions under the direct supervision of providers.
Strong computer skills and working knowledge of the windows-based environment.
Must have experience with electronic health record.
Bilingual in Spanish, and previous pediatric case management experience preferred but not required.

Physical Requirements:
Frequent sitting, occasional bending, stooping, lifting, reaching forward and overhead.
Almost constant standing and walking.
May occasionally be required to lift patients, office equipment and paper, usually not exceeding fifty pounds of direct lift.
Frequent computer work required.
Must be able to travel freely in the local area.
