Overview of Network
The Carolina Medical Home Network, LLC (CMHN) is a clinically integrated network owned and led by 34 Federally Qualified Health Centers (FQHCs) and the North Carolina Community Health Center Association (NCCHCA). There are 26 community health centers that are participating providers in the clinically integrated network (CIN). NCCHCA is CMHN’s sponsoring and managing partner. CMHN was formed in 2014 as a messenger model Independent Practice Association (IPA) and transitioned to a CIN in June 2020. Through their commitment to participate in CMHN, health centers are actively working to improve quality metrics in accordance with value-based contract expectations. With the advent of Medicaid managed care in North Carolina, CMHN transitioned from focusing on delivering healthcare services through its Medicare ACO to contracting with all Medicaid prepaid health plans and now serves more than 125,000 Medicaid beneficiaries. A state-of-the-art delegated care management program has developed to better serve high risk Medicaid patients as well as a growing expertise in referrals for patients needed social determinant of health services.

The Carolina Medical Home Network (CMHN) has released its request for proposal (RFP) to identify aligned Medicaid health plans to pursue value-based agreement terms that meet the specific needs of federally qualified health center (FQHC) and the complex patients they serve.
