Position Summary
As a member of the Care Team, the Behavioral Health Consultant (BHC) will provide support and mental health services to the NHC patient population. BHC will provide community resources and coordinate referrals as indicated.
Principal Duties and Responsibilities
1. Provide on-demand in-person integrated care consults to a diverse patient population in a fast-paced health center setting in collaboration with medical care team. Population can include pediatrics, family practice, geriatrics and/or prenatal patients. Consults provided from a biopsychosocial-spiritual framework. Services include screenings, support, brief interventions, and referrals as deemed appropriate.
2. Provide traditional counseling services and support to patients and family members.
3. Provide psychoeducational information as appropriate for patients and/or family members or accompanying caregivers
4. Contribute to the growth of the behavioral health department at NHC
5. Develop and maintain familiarity with related psychopharmacology
6. Collaborate alongside behavioral health team to ensure holistic health care and support of medical team
7. Provide training to staff as needed
8. Remain current and knowledgeable about local and regional resources for mental health and substance abuse treatment, developing collaborative relationships where possible
9. Participate in outreach educational activities in the community as needed
10. Maintain a regular schedule and caseload at assigned clinic in collaboration with treatment team
11. Participate in quality improvement projects as assigned
12. Participate in all meetings and trainings as assigned

Compliance and Documentation
1. Thoroughly, accurately, and promptly document all services rendered in the electronic medical record for every patient.
2. Be knowledgeable of and in compliance with CLIA, OSHA, HIPAA, and HRSA regulations for safety, infection control, equipment operation, confidentiality, and other applicable areas (e.g., patient identification and standard precautions).
3. Participate in proficiency testing as required by NHC or departmental policy to ensure that proper standards of care are being maintained.

Required Knowledge, Experience, or Licensure/Registration
1. Master’s Degree or PhD/PsyD in related Behavioral Health Field.
2. Current North Carolina LCSW(A), LMFT(A), or LCMHC(A) License or Licensed Psychologist in good standing.
3. Current CPR Certification.
4. HIPPA Compliance.
5. Ability to work onsite Mondays through Fridays from 8am to 5pm; to work remotely with secure Internet connection.
6. Bilingual ability preferred.
7. Experience in integrated care setting preferred.
8. Good physical stamina and an ability to stand most of the day.

Position Summary
Physicians provide professional comprehensive medical services for NeighborHealth Center (NHC) patients on a full-time basis as described in their employment agreement. As a member of a Care Team, the Family Medicine Physician with Prenatal Care works with the other care team members to provide quality patient-centered care.

Principal Duties and Responsibilities*
• Provides comprehensive primary care services to patients of all ages
• Offers routine family-centered prenatal care through delivery, with referral to OB partners for delivery; conducts postpartum follow-ups.
• Oversees and delivers NHC’s women’s health clinical services, collaborating with Advanced Practice Providers.
• Performs basic primary care office procedures, including women’s health procedures such as colposcopy and LARC (IUD, Nexplanon).
• Obtains health history and performs a physical exam to determine and execute a diagnosis and treatment plan in accordance with established standards, procedures, and practices. Requisitions lab tests and x-rays, writes prescriptions, and makes referrals as appropriate. Follows NHC clinical practice guidelines.
• Participates in the call schedule on a predetermined basis.
• Delegates patient care responsibilities that fall under the scope of practice of other Care Team members to those individuals (medical assistant, patient educator, registration representative, behavioral health).
• Attends staff and medical provider meetings and maintains a level of involvement with the health center committees, as needed, to ensure quality patient care. Participates in peer/chart review, performance evaluations, and quality improvement efforts.
• Provides patient care to approximately 9-10 patients per 4-hour shift per day as per productivity expectations for all providers.
• Collaborates with nurse practitioners or supervises physician assistants, as appropriate, in providing patient care; may assist in the education training of medical students and residents.
• Participates in continuing medical education, including Life Support Training, to maintain the skills necessary to perform physician duties and as required by applicable medical boards and the State.
• Maintains own physical health required to perform physician duties; maintains current immunizations and up-to-date PPD.
• Maintains communication with administration regarding personal issues related to drugs, felonies, discipline, loss of license or privileges, litigation where the provider or clinic is harmed or at risk, or other serious issues.
• Participates in quality improvement efforts on the Care Team.
• May perform other duties as assigned by the Chief Medical Officer.

Compliance and Documentation
• Confirms patient identity according to current procedure before rendering any service.
• Thoroughly, accurately, and promptly documents all services rendered in the electronic medical record for every patient.
• Performs all tests, procedures, and other services according to the policy and procedure of NHC, notify the appropriate supervisor in a timely manner of instrument or procedural problems.
• Is knowledgeable of and in compliance with CLIA, OSHA, HRSA, and JCAHO regulations for safety, infection control, equipment operation, and other applicable areas.
• Participates in proficiency testing as required by NHC or departmental policy to ensure that proper standards of care are being maintained
• Maintains confidentiality of patient health information in accordance with HIPAA and other applicable regulations.

Required Knowledge, Experience, or Licensure/Registration
1. Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited institution
2. Completion of residency program with board certification (or eligibility) in Family Medicine, and training and clinical experience sufficient to deliver quality prenatal care and office-based women’s health care (OB fellowship not required).
3. Board Certification (or eligible) in specialty training and maintenance of certification
4. State licensure to practice medicine in North Carolina
5. CPR Certification
6. Ability to work onsite/hybrid/remotely Mondays through Fridays during the hours of 8am and 5pm.

About Position: The primary responsibility of the Physical Therapist is to evaluate and reassess participants’ mobility level and other functional abilities. Provide and recommend therapeutic exercise, gait training, ambulation, etc. Plans and develops group exercise programs for maintenance therapy for participants. The Physical Therapist will also be responsible for the physical therapy provided at the Day Center to the geriatric population.

Duties –

Conducts an initial assessment and scheduled reassessment of all participants; communicates participant changes to team members.
Develops treatment program when indicated, sets goals and provides treatment.
Reviews and completes assessments and discharge summaries. Plans and develops maintenance, exercise and therapy programs for participants.
Provides manual muscle testing; sensory/perceptual testing; neuromotor functioning; appropriateness of current assistive/adaptive/orthotic devices.
Participates as a member of IDT; attends staff meetings and takes part in participant care planning; communicates any changes in participant condition to team members. Coordinates 24-hour care delivery. Provides training to family members, friends and PHSC staff members for those parts of the treatment plan that can be carried out by them. Emergency intervention, including mock codes.
Provides supervision to the program aides and other staff who are implementing a physical therapy treatment program.

Qualifications –

Must be able to demonstrate clinical competency with geriatric population.
Must meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently. Excellent interpersonal, oral and written communication skills.
Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

Position Summary:
Collaborative Care Management is an evidence-based behavioral health integration model designed to support primary care clinicians in assessing and treating patients with mild to moderate behavioral health conditions. The Collaborative Care Manager will serve as the central member and coordinator of the Collaborative Care team that includes the patient’s medical provider, clinical support and psychiatric provider, as appropriate, and will be responsible for maintaining a registry/caseload of patients to track and outreach patients while monitoring progression towards improvement. Position Summary

The Clinic Nurse is a registered nurse who is key to providing support for our providers, safe care for our patients, strengthening operational quality, and ultimately improving clinical patient outcomes. The Clinic Nurse assists in the care coordination of our patient’s healthcare needs, and serves as a teacher, advocate, and navigator for these patients within the context of the care team. This nurse will be responsible for providing direct nursing care, performing nursing assessments, participating in the continuity of patient care, being a servant-leader, and helping improve patient outcomes within the role of a clinic nurse.

Principal Duties and Responsibilities*

Provides clinical and administrative support for providers within the care team model. This includes:
Serving as a liaison between providers and their patients
Communicating abnormal lab results and treatment plans to patients as outlined by the provider and/or protocols
Providing patient education in areas such as chronic disease, medications, prenatal care, and wellness
Working with other members of the care team to coordinate patient care, referrals, DME orders, care management, and hospital/ER follow-up
Executing nursing care under the direction of the provider, such as: medication injections, wound care and dressing changes, and assisting in procedures
Navigating medication changes and prior authorizations due to insurance requirements under standing orders and the guidance of the provider
Obtaining pre-authorizations for diagnostic testing as required by insurance
Manages the clinical phone line. Assists pharmacies, responds to patient requests, conducts triage, and conveys messages to and from providers, clinics, specialists, and other care team members.
Directs the assessment of acutely ill patients who present to the clinic in person or via phone to ensure efficient workflow.
Provides nursing care to patients who present through the Nurse Visit Schedule. This may include, among other types of visits: BP checks, wound care, diabetes patient education, medication administration, triage, pregnancy tests/counseling, and conveying test results.
Collaborates with medical support staff and assists with prioritizing clinical tasks to ensure efficient workflow.
Assists with training and competency verification of medical support staff.
Ensures all emergency equipment and clinic use medications are properly stocked, stored, and logged (as applicable) for the care of patients at their clinic site.
Participates in care team meetings and quality meetings and other meetings as directed by the Chief Medical Officer.
Documents all activity appropriately in the Electronic Medical Record.

Job Summary
The Adult Medicine Nurse Manager will be responsible for overseeing the daily operations of the family medicine department. This includes managing a team of nurses, ensuring quality patient care, and maintaining compliance with all regulatory requirements.

Key Responsibilities
Manage a team of nurses, including hiring, training, and scheduling
Ensure quality patient care by monitoring patient outcomes and implementing best practices
Quality patient care by monitoring patient outcomes and implementing best practices
Maintain compliance with all regulatory requirements, including HIPAA and OSHA
Collaborate with physicians and other healthcare professionals to develop and implement treatment plans
Manage the department budget and ensure financial goals are met
Develop and implement policies and procedures to improve department efficiency and effectiveness
Participate in quality improvement initiatives and ensure departmental goals are met
Qualifications
Current RN license in the state of North Carolina
Bachelor’s degree in nursing or related field
Minimum of 5 years of experience in a healthcare management role
Strong leadership and communication skills
Ability to work collaboratively with physicians and other healthcare professionals
Knowledge of healthcare regulations and compliance requirements
Experience with budget management and financial analysis
