Requirements:

Graduate of an accredited Nurse Practitioner or Physician Assistant Program

CPR

Minimum of (1) year experience (preferred)

Responsibilities:

Staff Family Nurse Practitioner or Physician Assistant provides services to the clients at CommWell Health and in the community through outreach and community programs. Participates as a member of the Patient Centered Health Home (PCHH) Care Team.
