Agape Health Services is a federally qualified health center (FQHC) dedicated to providing comprehensive, high-quality healthcare to underserved communities in Eastern North Carolina. With a solid foundation in primary care, dental, behavioral health, pharmacy services, and chronic disease management, we are committed to expanding access to care, improving health outcomes, and reducing disparities in rural communities.
Nurse practitioners play a critical role in bridging healthcare gaps, especially in underserved areas. Their ability to provide patient-centered, comprehensive care contributes significantly to improved health outcomes and cost-effective medical practices. They are integral to shaping the future of healthcare delivery.
Key Responsibilities:
Patient Care: Conduct comprehensive assessments, diagnose medical conditions, and develop treatment plans. This includes addressing acute, chronic, and preventive care needs.
Prescribing Medications: Write prescriptions and manage pharmacological treatments based on patient needs and state regulations.
Diagnostic Duties: Order and interpret diagnostic tests such as lab work or imaging studies to inform clinical decisions.
Health Education: Counsel patients and their families about health conditions, disease prevention, and lifestyle modifications to promote overall well-being.
Collaboration: Work within interdisciplinary teams, partnering with physicians, specialists, and other medical professionals to deliver holistic care.
Documentation: Maintain accurate medical records, including patient histories, treatment plans, and progress notes.
