Care transitions coupled with unmet social needs are challenging and fraught with opportunities for complications and breakdowns, impacting patients, their families and care providers. During this webinar, with the help of subject matter experts we will explore the use of Health Information Technology to support effective care transitions that consider social drivers of health.  Attendees will have the opportunity to make recommendations in a panel discussion on how the Health Center Controlled Network (HCCN) can support community health centers meet these needs.
