Delivering value-based care in the era of COVID-19 increasingly requires the integration and effective use of data from multiple sources. Subject matter experts will describe initiatives underway to integrate data from electronic medical records (EMR), health information exchange (HIE), and health plan claims for care management and population health management.  We will also explore data security requirements related to data use in clinical care. Attendees will have the opportunity to make recommendations in a panel discussion on how the Health Center Controlled Network (HCCN) can support community health centers meet these needs.
